Membership
Jay Clinic is pleased to offer under the following terms and conditions a flat fee discounted physical health care service program known as “Jay Cares”. Please review the terms of this Agreement carefully including the attached Exhibit A –“Jay Cares Membership Plan Covered Items” which details the services included in the Jay Cares membership plan.
COST OF JAY CARES MEMBERSHIP PLAN.
Annual membership for the JAY Cares plan is $500 per individual.
The annual membership for a family is $500 per person x 3 family members then $250 per person after that. Membership begins on date of payment and continues for the following 12 calendar months. There is a nonrefundable processing fee of $75 per individual plan and $150 per family plan.
JAY CARES MEMBERSHIP PLAN COVERS THE FOLLOWING
Annual physical exam* by appointment (age and gender specific)
Unlimited office visits* (related to the conditions/services listed below)
Internet access to providers on weekends
Yearly Lab tests included: CBC, METABOLIC PROFILE 23, CHOLESTEROL PANEL, AND THYROID Blood tests. Onsite urinalysis.
Rapid Strep tests
Many other discounted lab tests
DOT and sports and camp exams.
Flu shots when available.
